Item Análise nutricional, social e concentração plasmática dos mediadores inflamatórios IL- 17, IL-33, CXCL16 e CCL2 em gestantes com toxoplasmose.(2023) Santos, Priscilla Vilela dos; Silva, André Talvani Pedrosa da; Andrade Neto, Valter Ferreira de; Silva, André Talvani Pedrosa da; Andrade Neto, Valter Ferreira de; Cupertino, Marli do Carmo; Aloise, Débora de Almeida; Figueiredo, Sônia Maria de; Ribeiro, Silvana Mara Luz TurbinoA toxoplasmose congênita é uma infecção causada por Toxoplasma gondii e relacionada a aspectos geográficos, culturais, socioeconômicos e, principalmente, ao padrão da resposta imune materno. Quando a mãe é infectada por T. gondii, geralmente não apresenta sinais clínicos e sintomas evidentes e, a exposição do feto acaba sendo negligenciada. Os bebês infectados congenitamente, podem manifestar perda da visão, microcefalia, hidrocefalia, retardo mental, surdez e epilepsia. Por se tratar de uma abordagem de suma importância, o presente estudo avaliou o perfil antropométrico, socioeconômico e sorológico para T. gondii em mulheres grávidas atendidas nas Unidades Básicas de Saúde e na policlínica de Ouro Preto, Minas Gerais, Brasil, correlacionando-o com hábitos alimentares, inflamatórios, socioeconômicos e clínicos. Este estudo transversal foi realizado entre abril e dezembro de 2020, incluindo 131gestantes na faixa etária entre 13 e 46 anos, avaliadas quanto aos níveis de anticorpos IgM e IgG anti-T. gondii e questionadas sobre os padrões socioeconômicos, ambientais, gestacionais, clínicos e alimentares. A prevalência de soropositividade para T. gondii foi de 45,8% (n= 60) e, nesse grupo, observou-se uma frequência maior de soropositividade em multigestas (p<0,009). Dentre as gestantes avaliadas, 37,40% encontravam-se no 2o trimestre da gestação atual, e o excesso de peso foi observado em 33,59% na amostra avaliada. A maioria das gestantes relatou ter ensino médio completo (58,02%), possuir trabalho (43,56%), possuir renda entre dois e três salários-mínimos (52,67%), residir na zona urbana da cidade (60,31 %) e ter acesso a saneamento básico como água encanada (86,26%), rede de esgoto (89,31%) e coleta de lixo urbano (97,71%) em sua residência. Em relação aos hábitos alimentares, a maior parte das gestantes consumia leite pasteurizado (89,31%), vegetais crus (99,24%) e carnes cruas/malcozidas (67,94%) e, no geral, 50,38% das mulheres relataram o hábito de limpar/lavar frutas e verduras em água corrente apenas. No contexto inflamatório, constatou-se que mulheres infectadas cronicamente apresentavam maiores concentrações plasmáticas de IL-33, CXCL16 e de CCL2 em relação ao grupo das gestantes não infectadas. Gestantes com sorologia sugestiva de infecção recente (IgM+/IgG+) apresentaram elevação na concentração plasmática de IL-17, sendo este perfil similar ao observado na infecção crônica (IgM-/IgG+). Como conclusão deste estudo sugere-se maior risco de transmissão congênita de T. gondii em mulheres multigestas, além de um potencial papel protetor para os biomarcadores plasmáticos IL-17, IL-33, CXCL16 e CCL2 nas fases inicial e crônica infecção em gestantes.Item Atividade tripanocida e anti-inflamatória de compostos derivados de 1,8-dioxo-octa-hidroxantenos.(2019) Menezes, Ana Paula de Jesus; Silva, André Talvani Pedrosa da; Costa, Guilherme de Paula; Costa, Daniela Caldeira; Sousa, Lirlândia Pires de; Andrade Neto, Valter Ferreira de; Veloso, Vanja MariaA infecção pelo Trypanosoma cruzi acomete milhares de pessoas pelo mundo e a eficácia dos fármacos disponíveis para eliminar este parasito é limitada à fase aguda da doença. A busca por moléculas mais efetivas contra o T. cruzi e de menor toxicidade para o hospedeiro é, portanto, uma prioridade para a saúde pública mundial. Neste contexto surgem as 1,8-dioxo-octahidroxanthenos ou xanthenodionas, compostos sintéticos derivados das xantonas que possuem inúmeras aplicações terapêuticas descritas, dentre elas atividades anti-inflamatória, tripanocida e cardioprotetora. Assim, o objetivo deste trabalho foi avaliar a ação tripanocida, in vitro, de compostos derivados das 1,8-dioxo-octahydroxantenos e sua influência, in vivo, na parasitemia e na alteração dos parâmetros inflamatórios durante a infecção murina. Para avaliar a toxicidade dos compostos in vitro utilizou-se testes colorimétricos (MTT) em formas tripomastigotas (cepa Y) e em macrófagos J774A.1, além de ensaios fluorimétricos (AlamarBlue) em células Vero com parasitos da cepa CL expressando uma proteína vermelho fluorescente (CLtdTomato) para determinação da CC50, IC50 e IS. Nesta etapa, doze compostos foram avaliados (MI17, MI18, MI24, MI25, MI39, MI45, MI68, MI73, MI78, MI79, MI80 e MI81) foram avaliados nesta etapa. O compostos MI80 (IC50=30.65) destacou-se por apresentar atividade anti-T. cruzi, com o melhor IS (170). Para avaliar in vivo o desempenho tripanocida e imunológico do composto MI80, camundongos Swiss foram infectados com as cepas Y (n = 10) e CL (n = 10) de T. cruzi utilizando um protocolo de tratamento por 10 dias com 50mg / kg de MI80. O MI80 reduziu 40% do pico de parasitemia em animais infectados com a cepa Y, enquanto reduziu, em ambas as cepas, o infiltrado inflamatório cardíaco. RankL, CCL2 e IFN-g foram mensurados no tecido cardíaco, mas o MI80 não foi capaz de modificar seu padrão em animais infectados com a cepa Y. No entanto, o MI80 aumentou o RankL e IL-10 em camundongos infectados com CL. Não foi observada toxicidade hepática e renal nos animais advindas do tratamento com o composto MI80. . Nossos achados mostraram que o 1,8-dioxo-octa-hidroxanteno tem um efeito anti-parasitário limitado e foi capaz de melhorar os parâmetros inflamatórios cardíacos relacionados à infecção por T. cruzi.Item Genetic polymorphism in IL17RA induces susceptibility to Toxoplasma gondii infection in Brazilian pregnant women.(2020) Andrade, Joelma Maria de Araujo; Oliveira, Claudio Bruno Silva de; Meurer, Ywlliane da Silva Rodrigues; Santana, Jéssica Emanuella; Almeida, Yngrid Gleyter Barbosa de; Santos, Priscilla Vilela dos; Souza, Débora Maria Soares de; Costa, Guilherme de Paula; Silva, André Talvani Pedrosa da; Palomino, Gustavo Martelli; Freitas, Janaina Cristiana de Oliveira Crispim; Andrade Neto, Valter Ferreira deCongenital toxoplasmosis is a parasitic disease caused by Toxoplasma gondii, an obligate intracellular parasite which can cause fetal death/abortion and can induce damage in the brain and eyes of the infected babies. The environmental and genetic factors associated with T. gondii and the maternal immune response, drive part of the pathogenesis of congenital toxoplasmosis. Thus, in this study, we aimed to investigate the allelic and genotypic frequencies of specific single nucleotide polymorphisms (SNPs) in the IL17A and IL17RA genes, as well as the production of IL-17A, IL-33, and CCL2 in pregnant women, from the State of Rio Grande do Norte, Brazil, further relating these along with the clinical parameters, to the toxoplasmosis infection. Through PCR-RFLP techniques, two SNPs implicated in Th17 immune response, IL17A rs2275913 (G> A) and IL17RA rs4819554 (A> G) modulation were evaluated in pregnant women, either infected or not infected by T. gondii. These women were also evaluated in terms of plasma release of CCL2, IL-33, and IL-17A which relate to hypertension, number of abortions, and ethnic pattern. The results showed that the G-allele of the SNP rs2275913 (IL17A) appeared to be protective in this population, while the rs4819554 (IL17RA) SNP G allele was associated with greater susceptibility to T. gondii infection [ρ value = 0.025; OR = 2.815 (1.118–7.089); CI = 95%]. None of the cytokines had any influence on the analyzed parameters (abortion and hypertension). In conclusion, our data suggest an immunogenic evidence of susceptibility to T. gondii infection driven by the rs4819554 (IL17RA) SNP G allele in Brazilian pregnant women. Further studies are needed to reinforce this trial marker in populations from distinct geographical areas as well as to confirm the protective pattern related to the G-allele of the SNP rs2275913 (IL17A) in pregnant women.Item Seroprevalence and risk factors for human toxoplasmosis in northeastern Brazil.(2018) Aloise, Débora de Almeida; Vital, Wendel Coura; Carneiro, Mariângela; Rodrigues, Marlus Venâncio; Toscano, Gislani Acásia da Silva; Silva, Ramiza Bernardino da; Andrade Neto, Valter Ferreira de; Vitor, Ricardo Wagner AlmeidaToxoplasmosis is one of the most prevalent parasitic infections in humans, causing severe clinical complications in immunocompromised patients or in foetuses. Estimates on the toxoplasmosis seroprevalence in human populations vary widely, even within the same region. This study aimed to estimate the prevalence of Toxoplasma gondii infection in the municipality of Santa Cruz, State of Rio Grande do Norte, Brazil, and identify the risk factors for toxoplasmosis. T. gondii IgG antibodies were measured in 66.2% of the studied population. The factors associated with infection were as follows: age above 45 years (OR = 7.4; 95% CI = 3.7-14.8); illiteracy (OR = 2.8; 95% CI = 1.6-5.0); the presence of more than three cats in the home (OR 2.0; 95% CI = 1.2-3.5); no water tank (OR = 2.0; 95% CI = 1.4-3.1); and consumption of raw or unpasteurized milk (OR = 2.0; 95% CI = 1.3-3.0). The risk factors identified were predominantly related to the ingestion of oocysts. This study may contribute to public health knowledge of toxoplasmosis and the definition of control programs, particularly for pregnant women from regions similar to the area studied.Item The imbalance in the relationship between inflammatory and regulatory cytokines during gestational toxoplasmosis can be harmful to fetuses : a systematic review.(2023) Santos, Priscilla Vilela dos; Toledo, Débora Nonato Miranda de; Souza, Débora Maria Soares de; Menezes, Tatiana Prata; Perucci, Luiza Oliveira; Silva, Zolder Marinho; Teixeira, Daniela Caldas; Vieira, Ed Wilson Rodrigues; Andrade Neto, Valter Ferreira de; Guimarães, Nathalia Sernizon; Silva, André Talvani Pedrosa daObjective: To evaluate the available information on inflammatory and regulatory plasma mediators in pregnant women (PW) diagnosed with toxoplasmosis. Source: The PubMed, Embase, Scopus, and Lilacs databases were evaluated until October 2022. Study eligibility criteria: This review was carried out following the PRISMA and registered on the PROSPERO platform (CRD42020203951). Studies that reported inflammatory mediators in PW with toxoplasmosis were considered. Evaluation methods: After excluding duplicate articles, two authors independently carried out the process of title and abstract exclusion, and a third resolved disagreements when necessary. The full text was evaluated to detect related articles. The extraction table was built from the following data: Author, year of publication, journal name and impact factors, country, study design, number of gestations and maternal age (years), gestational period, diagnosis of toxoplasmosis, levels of inflammatory markers, laboratory tests, and clinical significance. Methodological quality was assessed using Joanna Briggs Institute tools. Results: Of the 1,024 studies reported, only eight were included. Of the 868 PW included in this review, 20.2% were IgM+/IgG- and 50.8% were IgM-/IgG+ to T. gondii, and 29.0% uninfected. Infected PW presented higher plasma levels ofIL-5, IL-6, IL-8, IL-17, CCL5, and IL-10. Regarding the methodological quality, four studies obtained high quality. Data from this review pointed out the maintenance of the inflammatory pattern during pregnancy with a closely related to the parasite. Conclusion: Immune status in PW defined the course of the T. gondii infection, where the equilibrium between inflammatory and regulatory cytokines mitigated the harmful placenta and fetus effects.Item Upregulation of IL-33, CCL2, and CXCL16 levels in Brazilian pregnant women infected by Toxoplasma gondii.(2023) Santos, Priscilla Vilela dos; Toledo, Débora Nonato Miranda de; Guimarães, Nathalia Sernizon; Perucci, Luiza Oliveira; Andrade Neto, Valter Ferreira de; Silva, André Talvani Pedrosa daCongenital toxoplasmosis can cause neurological and eye damage, behavioral alterations, or death in fetuses or babies born to Toxoplasma gondii-infected women. Several pieces of evidence suggest that socioeconomic, environmental, and inflammatory patterns linked to the maternal immune response partly drive the pathogenesis of this disease. However, immunoregulation induced by T. gondii infection during gestation is not completely understood. The aim of this study was to assess the association between T. gondii seropositivity and concen- trations of plasma markers (CCL2, CXCL16, IL-17, and IL-33) in Brazilian pregnant women. Inflammatory markers were measured by immunoassays in the plasma of 131 pregnant women (13 to 46 years old). The prevalence of T. gondii infections was 45.8% (n = 60) in this population. The concentrations of CCL2, CXCL16, and IL-33 were higher in T. gondii-seropositive than in seronegative pregnant women, while the opposite was observed for IL-17 levels. In IgG+ women, a strong correlation between IL-17 and IL-33 (r = 0.7508, p = 0.0001) and a moderate correlation between CXCL16/IL-17 (r = 0.7319, p = 0.0001) and CXCL16/CCL2 (r = 0.3519, p = 0.0098) was observed. In uninfected women, a strong correlation was found between IL-17 and CXCL16 (r = 0.6779, p = 0.0001) but moderate between IL-17 and IL-33 (r = 0.4820, p = 0.0001). In summary, our data suggest that plasma upregulation of CCL2, CXCL16, and IL-33 might exert a potential protective role in the mother/fetus/parasite axis and, in addition, multiparous women are more likely to be infected with T. gondii than primiparous women.
